In the United States Lasik and similar laser eye surgery techniques are approved for patients over the age of 18. What is more important is stability in refractive error. You need to have had two consecutive years with no more than a 0.25 diopter change in prescription.

Persons under the age of 18 have received laser eye surgery, but these are very rare cases and would be considered appropriate only when the patient has a very high refractive error that might interfere with normal vision development.

Also if I have medical insurance will it pay for all or any of it? I'm still a teenager and I've worn glasses half of my life.. I absolutely hate them.. I've tried putting contacts in for 3 years and I just CAN'T. Also how much is it or does it depend on how bad your eyes are?

you have to be 18 for any elective surgery
above that, for lasik surgery you need to have had a stable prescription for a couple of years (i've heard some won't even consider it til you're 21)
as it's a technically 'unecessary' procedure (in the same way as body enhancement such as boob jobs) no medical insurance is likely to cover you, i'm afraid
cost is generally fairly standard across the board, but for a tailored quote you'd need to go to a few companies and compare costs/sergices
